If you're a runner in Brisbane building toward an event and you've started to feel a dull, aching pain along the inside of your shin that shows up early in a run and eases once you're warmed up, you're most likely dealing with shin splints, a common overuse injury in running. Left unmanaged, it has a habit of resurfacing every time training load goes up again.

What Causes Shin Splints?

Shin splints, or medial tibial stress syndrome, happens when the load going through the shin bone and the muscles and connective tissue attached to it outpaces what those tissues can currently handle. It's rarely one single cause. The common pattern is a training change. A jump in weekly distance, a return to running after time off, more hill or hard-surface running than usual, or a change in shoes. Reduced calf and tibialis strength, foot and ankle biomechanics, and running cadence can all shift how much load lands on that part of the shin. Most cases are genuinely multifactorial, which is why a generic "stretch more" answer rarely holds for long.

Why Shin Splints Need a Proper Assessment, Not a Guess

The tricky part with shin splints is what it can be confused with. Ordinary shin splints and a bone stress injury (a stress reaction or stress fracture in the tibia) can feel similar early on, but they are not the same thing and they are not managed the same way. Location matters. A 2023 systematic review and meta-analysis of nearly 3,000 bone stress injuries found that injuries at the back and inside of the tibial shaft (roughly where shin splints pain sits) have a strong prognosis, with close to 98% of athletes returning to sport in an average of around six weeks. Injuries at the front of the tibial shaft behave very differently, with a substantially lower return-to-sport rate and a higher complication rate (Hoenig et al., British Journal of Sports Medicine, 2023).

That difference is exactly why self-diagnosing off a symptom checklist is risky. It's also why "just rest it" isn't automatically the safe option. Rest reduces symptoms temporarily but doesn't rebuild the load tolerance needed to run without it recurring, and on its own it won't tell you which of these two different injuries you actually have.

Training & Load History

We start with what's changed recently in your training whether it be mileage, intensity, surface, footwear and any prior injury. This is often where the real driver is hiding.

Running Gait Assessment

Treadmill-based gait analysis identifies cadence, foot strike pattern and loading mechanics that may be contributing to the injury. This is tied directly into your training plan, not assessed as a one-off curiosity.

Strength and Load Tolerance Testing (VALD)

Isometric strength testing of the calf and tibialis gives an objective baseline for where your capacity currently sits and how far it needs to progress before higher-load running is appropriate.

Evidence-Based Treatment for Shin Splints

Treatment follows the same framework as the rest of your care at SANO: Restore, Function, Perform.

RESTORE: The early focus is settling irritation, usually through load modification rather than complete rest, and hands-on treatment where it helps you tolerate the next stage of loading more comfortably.

FUNCTION: This is where the real work happens with progressive strengthening of the calf and tibialis, addressing training and biomechancial factors from your assessment, and rebuilding tissue capacity in a structured, monitored way.

PERFORM: A structured return-to-running program, with reassessment as you progress, so training volume goes back up in a way your shin can tolerate, with an eye on preventing the same pattern repeating in the next training block.

When Should You See a Physiotherapist?

If shin pain hasn't settled within a week or two of easing training, or keeps recurring every time you build volume back up, it's worth getting assessed rather than guessing. A few signs point toward something needing more urgent attention. A pain sharply localised to one spot rather than spread along the shin, pain at night or at rest, or pain that gets worse despite reduced training load. These can indicate a bone stress injury rather than straightforward shin splints and changes how it needs to be managed.

Common Questions about Shin Splints

How long do shin splints take to heal?

Mild cases often settle within a few weeks of appropriate load management and strengthening. More established or recurring cases typically need a longer structured program to properly rebuild capacity, not just reduce symptoms.

Can I keep running with shin splints?

Often yes, in a modified form with reduced volume or intensity rather than a full stop. This depends on what your assessment finds. That's exactly why a proper assessment matters, rather than pushing through or stopping entirely on your own.

Do I need a scan?

Not always. Imaging is more relevant when there's a suspicion of a bone stress injury rather than straightforward shin splints. Your physiotherapist will advise if it's warranted based on your assessment findings.

Will shin splints come back?

It can, particularly if the training load or a biomechanical factor caused the injury in the first place isn't addressed. That's the reasoning behind the Perform Phase and an ongoing Running Assessment relationship, rather than treating the current flare-up in isolation.
